Illustrative healthcare workflow — clinical decisions remain with authorised professionals.

Connecting the patient journey to the work behind it.

Patient information needs to reach the people who can act on it. When questionnaires, scoring and provider applications sit in separate processes, patients and staff can be left bridging the gaps. We design connected workflows around the needs of both.

Our experience includes digital patient journeys, provider applications, clinical questionnaires and scoring, FHIR, HL7 and EHR integration. We start with the workflow, the information it needs and the clinical responsibilities that must remain clear.

Make the next step clear for patients and providers

Design the journey from a request for information through completion, review and follow-up. Patients need understandable questions, accessible interactions and a clear account of what happens next. Providers need the relevant information in the context of their work.

Map who receives a response, who checks it and how missing or incomplete information is resolved. Test with representative users and include a route to human support, so a completed form leads into an owned process.

Connect questionnaires, scoring and review

Treat a questionnaire, its scoring method and the provider workflow as connected parts of the service. Agree which version is in use, the required inputs and how the result is presented for review. Keep the source responses available alongside the score and its context.

Use explicit rules for agreed calculations and routing. Where intelligent assistance is considered, define its permitted role, evaluation and escalation before use. Clinical interpretation and decisions remain with appropriately authorised professionals.

Design interoperability around the information being shared

We connect healthcare systems around the information that needs to move between them, using standards such as FHIR and HL7.

We make ownership, access and traceability clear from the start, so information can move reliably while meeting the security and governance needs of the organisation.

Measure the workflow with the people using it

We establish how the process performs today, then combine operational measures with feedback from patients and providers to see where change is making a difference.

We start with a defined journey, learn from how it works in practice and extend it carefully as the evidence becomes clearer.

Experience at Gillette Children’s

For Gillette Children’s, we built an integrated digital workflow spanning patient questionnaires, clinical scoring and provider applications, connecting into the wider healthcare environment through FHIR, HL7 and Cerner.

This is experience of joining patient information to provider workflow. It gives a concrete starting point for discussing your own environment, whose clinical processes, interfaces and controls need their own discovery and design.

Solution

Connect Fragmented Systems

We review how the service is performing with the people closest to the operation, then use that insight to decide where change will have the most value.
